If you don’t have health insurance—or choose not to use your insurance—you have the right to receive a Good Faith Estimate of the expected cost of your scheduled, non-emergency medical care.
A Good Faith Estimate helps you understand the anticipated charges before your visit.
Your rights include:
- Receiving a written Good Faith Estimate for eligible services.
- Requesting an estimate before scheduling your appointment.
- Disputing a bill if it is $400 or more above your Good Faith Estimate.
Please note that estimates only include services provided by Auburn Family Health Center. Charges from outside laboratories, hospitals, imaging centers, pharmacies, or other providers are billed separately.
To request a Good Faith Estimate, call 402-274-4993.
For additional information about your rights under the No Surprises Act, visit the Centers for Medicare & Medicaid Services (CMS).
