Information for New Patients
- Please arrive 15 minutes before your scheduled appointment to complete the check in process.
- Bring your photo ID and insurance card(s)
- Bring your medications (preferred) or a CURRENT list of your medications including dosage information. This includes but is not limited to
- Prescription medications
- Over the counter medications
- Ointments and creams
- Eye/Ear drops
- Inhalers
- Injections
- Vitamins/Supplements
- Bring a copy of your previous medical records (if possible) along with your family health history and your immunization history. If you have a complex medical history, a detailed list of diagnoses/testing/procedures is helpful.
